I have had the opportunity to work alongside Chilwell house rehabilitation unit in my role as lead nurse within the Early Intervention team.
The team supported a careplan to allow for one of my clients to discharge from hospital after a lengthy admission. Over time they made efforts to not only support the transfer but also to set and review moving on goals. This enabled my client to shift from being somewhat inactive and lacking any plan or structure to his day to having positive thoughts about moving on with his life.
Aspire Mental Healthcare offers integrated community based services aimed at promoting recovery and independence.Our Supported Living service offers accommodation to those with mental illness, able to live semi-independently, but who require either long-term supervision or practical assistance with certain life-skills or short-term rehabilitation before returning to live back at home.
Individual support packages are designed around the specific assessed needs of the service user.
Range of supportThe level of support depends on individual needs and includes:
- Tenancy support
- Support with personal care, domestic activities, daily living, social and leisure activities.
- Medication administration
- Step down from hospitals
