They will provide feedback on whether they feel Bramble House will be able to meet the needs of the individual. A proposal of care will then be submitted and if the individual and their care team are happy to proceed, admission and transition plans will be agreed.

Caring for elderly individuals with serious mental illnesses requires a comprehensive, individual-focused approach that addresses their mental, cognitive, physical, and social needs. Key aspects of care include regular mental health assessments, managing medications, and providing education for both patients and caregivers. A structured routine helps promote stability, while engaging in social activities can help reduce feelings of isolation. It’s essential to incorporate evidence based therapies, such as cognitive-behavioral therapy, and to have support from multidisciplinary teams that include social workers and nurses. Creating a safe environment and ensuring access to community resources and support groups are crucial for improving their quality of life.
